Hey parents and students! It's that time of year again where we need to get all of our paperwork in order for the upcoming season. To ensure that we are all set to hit the ground running, there are a few documents that need to be filled out annually.

First up, we have our OHSAA Pre-Participation Evaluation forms, or physicals as they are commonly called. These forms are require for us to have on file and contain all of the necessary information about each player, including emergency contacts and medical history. It's important that these forms are filled out completely and accurately to ensure the safety and well-being of all of our athletes. These are required before any athlete can particpate in mandatory practices once the season has begun.

Next, we have our Emegency Medical Authorization. EMAs are a quick reference form held by a coach during gametime that has vital medical information about the athlete and emergency contact information. These are used  in the case that a player is injured on the field and needs fast medical attention. These EMAs are part of the PPE packet listed above. In some cases, EMAs get missed out of the above packet. This is just a handy download in case you need another. necessary to protect both our organization and our players in the event of any accidents or injuries that may occur during practices or games. It's important that all players and their parents or guardians sign these waivers before participating in any activities.

Lastly, we have the Transportation Form. We encourage our players to ride the bus to and from our athletic competions. This helps build camaraderie between teammates. We also understand that sometimes a parent may not be able to get back to Lehman to pick up their athlete. In this case, the Transportation Form needs to be filled out by the parent and signed by the JRH Athletic Director or any of the three principals (PCS, SP, or HA). This form is good for only one event and MUST be turned in prior to the bus leaving for the event. Post-game can get very chaotic for the coaches when it comes to making sure all players are safe and accounted for. When parents take their players without the coaches knowledge or attempts to turn in forms after the game, it only adds to the chaos and increases the chances of an athlete being left behind.  For this reason,  once the bus has left for the event, no forms will be accepted and the athlete  WILL return to Lehman on the bus, no exceptions.
